- Legal name and address of the applicant organization
- Contact person and telephone number
- Date of establishment, brief history, and mission statement
- Clear description of the program or project proposed for funding, substantiation of need, and amount requested
- Unique aspects of the program in relation to other similar programs in the area
- Copy of the most recent letter from the Internal Revenue Service determining 501(c)(3) tax-exempt status
- Other funding sources and amounts
- Copy of the most recent year-end audited financial statements (unbound) and a copy of IRS Form 990
- Current income and expense budget for the agency (and the program if applicable)
- Qualifications of the individuals administering the project
- One page list of the names and occupations of the Board of Directors and officers
- Actual number of clients served by the agency during the most recently completed year, the cost per client served, the total number of staff (full time and part time), and the total number of volunteers.
The Board of Trustees of the A. V. Hunter Trust, Inc. meets to review grant proposals three times per year. Please call the office for specific deadlines. The Trust does not accept proposals by facsimile or electronic transmission.
In order to evaluate the impact and effectiveness of the Trust’s funding, all grantees must submit a post-grant evaluation that includes the specific use of funds and an overall assessment of the program or project funded.
