Individual Application
Requests on Behalf of Individuals
The A. V. Hunter Trust provides funding to legal residents of Colorado for the purchase of durable medical equipment (DME).
The focus of this fund is to assist low-income, working poor and those who have no other resources available for the procurement of DME. All requests must be made by an unrelated third-party on behalf of the individual in need. Please note that the Trust will not reimburse vendors or clients for previously acquired medical equipment or services
Individuals who contact the A. V. Hunter Trust directly or who fill out their own application form are automatically disqualified from funding.
Special Note to Caseworkers:
Please do not submit incomplete applications. All sections must be completed in full or the application will not be processed.
- Total Household Income includes all income from anyone living in the home.
- Applicant’s Background Information and Personal History. If the applicant is disabled, you must enclose a doctor’s letter including diagnosis and prognosis. If there is a history of substance abuse, you must include documentation of one year’s sobriety and on-going counseling.
- All requests must include clear copies of two forms of identification including a picture ID such as a Colorado Driver’s License or Colorado ID; and a Social Security card. Immigrants must be legal residents and able to provide a copy of their Naturalization Papers or Green Card.
- A copy of the Prescription, Vendor Estimate, and/or Dental Treatment Plan must be included with all applications.
All applications should be mailed to: A. V. Hunter Trust, Inc., 650 S. Cherry St., Ste. 535, Glendale, CO 80246. Do not send by Certified or Registered mail. You will receive notification as to whether or not your client has been approved for funding. The vendor/dentist will be notified only if funding is approved. It is your responsibility to inform your client in a timely fashion. Remember, each funding commitment has a deadline for completion.
Vendors must provide all equipment or services and submit the signed request for payment with an invoice prior to the deadline in order to be paid.
